It is my intention to provide you with sensitive, compassionate, and quality therapy sessions. I am committed to offering my services at a competitive rate, and in circumstances where the need is merited, I am able to offer our services on a sliding scale basis. My ability to keep my rates affordable and to offer a sliding scale is due in large measure to the responsibility of persons like yourself. Payment is expected at the time of service. Payment in a timely manner allows me to keep my overhead low and with that, my rates are able to remain competitive and affordable. Because the appointment time is reserved for you, a FULL FEE may be charged for cancellations that occur within 24 hours of a scheduled appointment or for a missed appointment. A credit card is required and will be kept on file in the event of this occurrence.

$125 for a 45-minute session
$150 for a 60 -minute session
Initial session is typically 1 hour, with subsequent sessions lasting 45 minutes.

Services may be covered in full or in part by your health insurance or employee benefit plan. I am currently paneled with;

• Blue Cross Blue Shield
• Aetna
• United Behavioral Health

I can also provide the necessary invoice and documentation as an out-of-network provider that you can file with your individual Insurance plan.

Prior to your first appointment, I request that you please check your coverage carefully in order to avoid any obstacles in receiving the help that you are looking for. You can do this by calling the phone number on the back of your insurance card. Please verify with your individual plan, that I am in-network. You are responsible for charges according to your plans coverage.

Some questions to ask your carrier can include:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?

Is approval required from my primary care physician?

All payments are made directly from your health portal account with the credit card/HSA card that you place on file.

We accept all major credit cards, Flex Spending and Health Saving account debit/credit cards.(3 % processing fee).

If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
